  • Call a benefits counselor at 1-866-360-1934,
    8 a.m.–8 p.m., 7 days a week (TTY for the hearing impaired 1-800-662-1220) to make an appointment for a home visit.
  • Or enroll right over the phone.

Elderplan is a Medicare Advantage organization with a Medicare contract. Anyone entitled to Medicare Parts A and B, living in Brooklyn, Manhattan, Queens, Staten Island or the Bronx, may apply.9, 10 Enrolled members must continue to pay their Medicare premiums if not otherwise paid for under Medicaid.
