Who We Are

For more than 40 years, we have been helping New Yorkers access the care, support, and resources they need to live healthier, more independent lives.

As a not-for-profit health plan, our focus is simple: putting our members first. We believe health care should be personal and centered around each individual’s unique needs, goals, and what matters most to them.

Through our Medicare Advantage and Long-Term Care plans, we support older adults and individuals eligible for both Medicare and Medicaid. Beyond coverage, members have access to care management, wellness programs, and valuable benefits designed to support their health, independence, and overall well-being. Many plans also offer additional programs and services that help members stay active, engaged, and connected to their communities.

As a participating agency of MJHS Health System, we are uniquely positioned to connect members with additional health care resources, clinical expertise, and supportive services backed by decades of experience caring for older adults and families throughout New York. This connection helps ensure members have access to coordinated support as their needs change over time.
