FIDA Participant Materials
If you need detailed information about Elderplan FIDA Total Care, please consult our library of plan materials. (You will need Adobe Reader, a free download from Adobe Systems, to read these items).
We must provide information in a way that works for you (in languages other than English that are spoken in the plan service area, in Braille, in large print or other alternate formats, etc.).
For your convenience, you can now have your plan materials delivered straight to your door or email account via our secure

You also have the option to opt-in to receive all your materials electronically going forward. You will be asked to enter in your Member Id number located at the back of your Member Id card. If you have trouble logging in, a Participant Services representative will be happy to help you.
If you want a Provider/Pharmacy Directory mailed to you, or if you need help finding a network provider and/or pharmacy, please call Member Services at 1-855-462-3167. You may also email your request for the directory at firstname.lastname@example.org. You can always access our online directory at find a provider or pharmacy.
We have free interpreter services to answer any questions you may have about our health or drug plan. To get an interpreter, just call us at 1-800-353-3765. Someone who speaks your language can help you. This is a free service.
