Eligible Screenings and Activities
| Stay on top of your health and earn rewards by completing the following. | |||

| ✔ | Annual Primary Care Physician (PCP) Visit | In-office, in-home, or via telehealth | |
| ✔ | Flu Shot | A flu vaccination received in 2027 | |
| ✔ | Retinal Eye Exam | Completed in 2027 | |
| ✔ | Mammogram | Breast cancer screening completed between October 1, 2024, and December 31, 2027 | |
| ✔ | Colorectal Cancer Screening | Screening completed within the last 9 years. (Includes at-home Fecal Occult Blood Test) | |
| ✔ | Diabetes Screening | A hemoglobin A1c test to screen for diabetes and prediabetes | |
| ✔ | Kidney Screening | Includes both an eGFR and a uACR test to check kidney function | |
At-Home Screening Options
Some screenings can be completed in the comfort of your home, including:
- Diabetes screening
- Kidney screening
- Colorectal cancer screening (FOBT)
- Retinal eye exams

How to Get Rewarded

Bring your Wellness Incentive Form to your provider visit.

Complete your screenings and receive recommended vaccinations.

Have your health provider sign and stamp the form in the space provided.
How to Submit Your Form
Submit your completed form as soon as possible, no later than December 31, 2027:
