We value the health and wellness of our members and are excited to launch a wellness incentive program that will enable you to be rewarded with $25 gift card for each completed screening you are eligible for and have received by December 31, 2022. All you have to do is take care of yourself. Members have until 1/31/2023 to respond to the 2022 program.

How to get your $25 gift card:
Screenings
- During your next visit with your Assessment Nurse, you will be asked whether you have completed a flu shot, dental or vision exam in the last year or a hearing exam in the last two years. If the answer is yes to any of these screenings or vaccination, once verified, you will be rewarded with a $25.00 gift card for each one completed.
- Already had your annual visit with your Assessment Nurse and completed these screenings after that visit? Congratulations on taking good care of yourself. Please contact a member of the Wellness team and we will send you a $25.00 gift card for each eligible screening you completed.
- Not yet scheduled for your annual visit with your Assessment Nurse and would like assistance with coordinating any of these screenings? We’d be glad to help. Please contact a member of the Wellness team and they will help you schedule the screenings, most of which are offered in the comfort of your own home.
Covid Vaccine and Booster
Simply send us a copy of your completed Covid vaccination card showing both doses of the Pfizer/Moderna vaccine, or the single dose of Johnson & Johnson and, if applicable, the booster shot. There are three easy ways to provide your completed vaccination card. Pick the one that’s best for you:

Note: You can only receive one gift card for the original Covid vaccination. If you submit for your booster shot and have already received a gift card for the initial vaccine, you will receive only one $25 gift card.
Gift cards will be sent out after we verify that each screening has been completed. Please allow up to 14 weeks for processing. You must have continued enrollment in the Plan in order to be eligible for the reward.
