Activate Your Elderplan Over-the-Counter (OTC) Card

In order to start using your OTC Card, you must first activate it.

Follow these easy steps:

1. Go to the OTC member portal. You will be asked to provide the following information:
   • The 19-digit OTC card number on the front of the card
   • Your Elderplan Member ID number
   • Your birthdate (two-digit month, two-digit day, and four-digit year; for example, 05/20/1955)
2. You are ready to start using your Elderplan OTC Card!
