Therapeutic Leave
Therapeutic leave covers overnight stays away from your skilled nursing facility (SNF) or assisted living facility (ALF) where you currently reside. Your Elderplan Advantage for Nursing Home Residents (HMO-POS I-SNP) plan includes a Therapeutic Leave benefit so you can spend time with family, attend a special event, or simply have a change of scenery — without losing your bed or your benefits.

What Counts as Therapeutic Leave?
- Family events (weddings, holidays, birthdays)
- A short visit home or with a relative
- Medical or specialty appointments that involve an overnight stay
- Religious holidays or community events
How to Plan a Therapeutic Leave
- Talk to your facility’s care team as soon as you know the dates
- Call your Elderplan Care Team at 1-800-353-3765 (TTY: 711) to coordinate
- Confirm your medications — your pharmacy or facility will prepare what you need
- Plan transportation — your $1,000/quarter transportation benefit may help.
