Helping You Understand Your Benefits & Plan Your Next Step
We partner with Senior Planning Services (SPS) to help you understand your public benefits, complete applications, and make informed decisions about long-term care and community programs. SPS provides one-on-one guidance so you can confidently plan for your future.
- Applying for assistance programs you may qualify for
- Understanding the paperwork or letters you have received about public assistance or benefit programs
- Talking through your or public benefits and options when you’re not sure where to start

Connecting You with Community Resources for Everyday Needs
The Social Care Network (SCN) connects eligible Medicaid members with community organizations that provide services to address health-related social needs. A Social Care Navigator can help you access local programs and support.
A Social Care Navigator will walk you through your options for:
- Food and nutrition services
- Housing and utility support
- Transportation
- Employment support
- Care management to help coordinate your services
