Social Care Network (SCN)
Connecting You with Community Resources that Support Your Everyday Needs
As a New York Medicaid Member, you have access to a regional Social Care Network — a state program that connects you with free help for the everyday things that affect your health. That includes a safe place to live, cooking supplies to support healthy cooking practices, a way to get to your appointments, and someone to help coordinate it all. These services are known as Health-Related Social Needs (HRSN) services.
The Social Care Network is part of New York State’s Medicaid program. Elderplan/HomeFirst partners with 5 regional Social Care Work Lead Entities across our services areas- the 5 boroughs of New York City, Nassau, Westchester, Rockland, Putnam, Sullivan, Ulster, Durchess and Orange counties.
Who can use the Social Care Network?
Getting Help Through the Social Care Network
Social Care Network services are available to New York Medicaid members who qualify for Health-Related Social Needs (HRSN) services. To find out if you qualify, you’ll first complete a short screening about your needs.
You can complete the screening with your Care Manager, your health care provider, or a Social Care Network representative.
This includes members of:
- Elderplan for Medicaid Beneficiaries (HMO-POS D-SNP)
- Elderplan Plus Long-Term Care (HMO-POS D-SNP), our Medicaid Advantage Plus (MAP) plan
- HomeFirst Managed Long-Term Care (MLTC)
Not sure if you have Medicaid? Call Member Services, and we can help you find out.
