Health Care at Home

Services That Help You Live Well at Home

Health care at home also referred to as Long-Term Services and Supports (LTSS) are  services that help you remain safe, healthy, and independent in your home and community. Whether you need skilled care, help with everyday activities, or support for your caregiver, HomeFirst is here to help you get the services you need.

The services below are covered by HomeFirst when they’re medically necessary, included in your plan of care, and approved through a service authorization. If you think you may benefit from one of these services, contact your Care Manager. Your Care Management Team will assess your needs, coordinate any required approvals, and help arrange your care.
