What Your Team Does
Your Care Management Team is here to help coordinate your care and connect you with the benefits, services and support you need.
They:
- Develop a care plan with you based on your needs, goals, and preferences.
- Coordinate and arrange your covered services.
- Stay in touch with your doctors and providers to help ensure your care is coordinated.
- Help you find providers and schedule appointments.
- Visit you at home at least once a year, and whenever your needs change.
- Help you apply for or renew benefits, including Medicaid.
- Connect you with community resources and support programs.
Your Care Plan
Your care plan is officially called a Person-Centered Service Plan (PCSP), but you can think of it as your personalized care plan.
It outlines the services and support you receive and is built around what matters most to you, including your health needs, personal preferences, culture, family, and daily routines. Your care plan is reviewed regularly and updated whenever your needs change.
