See Your Health Information in the App of Your Choice
Connect to Your Health Information
It is easier than ever to view and manage your health information using apps on your phone, tablet, or computer. You decide which app to use and what to share. We make sure your data gets to you safely.
What you can see
HomeFirst pays for your long-term care services. When you connect an app, you can see the information we have about those services.
Your long-term care services
- Services HomeFirst has paid for, such as home care visits, personal care, and adult day health care
- The dates you received each service
- A short description of each service
- Medical equipment and supplies HomeFirst has paid for
Your service requests:
- Whether a request for a service is waiting for a decision, approved, or denied
- The reason for a denial, if there was one
Service Authorization information:
- The status of any prior authorization request — pending, approved, or denied
- The reason for a denial, if there is one

Apps to consider
You can connect any app that meets the federal security standards. Here are some that already work with HomeFirst:
This isn’t a complete list. HomeFirst doesn’t endorse or recommend any specific app — the choice is up to you. Always read an app’s privacy policy before you connect it. We do a technical security review on every app and won’t connect any that fall short of the required standards.
Keep Your Privacy in Mind
- Once you tell us to share your data with an app, that app controls how it uses, stores, or passes on your information. Federal privacy rules (HIPAA) protect your data while we hold it, but those same rules usually don’t apply once it’s with a third-party app. That’s why it’s so important to pick an app you trust and to read its privacy policy first.
- Third-party apps aren’t part of HomeFirst and may not be covered by HIPAA. If an app misuses your information or doesn’t follow its own privacy policy, you can report it to the Federal Trade Commission (FTC).
If you have a concern about privacy practices
If you believe HomeFirst hasn’t properly protected your health information under HIPAA, you can:
- File a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
- Contact Elderplan/HomeFirst Privacy Officer at 718-759-4260.
- File an anonymous complaint through the Elderplan/HomeFirst Privacy Hotline at 855-395-9169 or online at the Elderplan Compliance & Ethics Hotline.

Tips
Keeping Your Information Safe
Choose apps carefully.
Before connecting an app to your health information, review its privacy practices. Make sure you understand what information it collects, how it uses or shares your data, and what happens to your information if you stop using the app.
Read the app’s privacy policy.
Look for a policy that is clear and easy to understand. Check how the app will notify you if its privacy practices change.
Know what information the app collects.
Some apps may collect information that is not related to your health, such as your location.
Keep your username and password private.
Don’t share them or write them where others can see them.
