How Your Provider Network Works

Know Your Options Before You Get Care

Getting the care you need should feel simple. This page explains how your HomeFirst provider network works, so you know who to see and how to get started.

A provider network is the group of professionals and agencies that HomeFirst partners with to care for you. It includes nurses, home care helpers, therapists, and other providers who deliver the services your plan covers. We carefully choose and check every provider in the network, so you can feel confident you’re in good hands.

For the services your plan covers, you’ll use providers in your HomeFirst network. And you’re never on your own; your Care Management Team helps arrange your services, set up appointments, and make sure everything works together.

4 Things to Know About Your Provider Network

1. You keep your own doctor2. No plan premiums, copays, or deductibles

You don’t have to switch doctors when you join HomeFirst. Your own doctor stays your doctor. Your care team will work with them to plan your care.
There are no monthly fees, copays, or deductibles. As long as you use our network and follow the steps to get care approved, you don’t pay anything for what HomeFirst covers.
3. No referrals needed4. Care that comes to you
You don’t need a referral from your doctor to get HomeFirst services. Your care team helps set things up. Most services do need to be approved before they start — but we usually take care of that for you. Many services can come right to your home — like nursing, personal care, and meals. We can also help you access programs in your community, such as adult day programs. You can also talk to your doctor from home.

Many visits can happen by phone or video. This is called telehealth.

Service Authorization

Most long-term care services require approval before they can begin. This approval is called a service authorization. If you think you may need a covered service, contact your Care Manager. Your Care Management Team will assess your needs, coordinate any required service authorizations, and help arrange the care that’s right for you.

Continuing Your Care

We want your transition to HomeFirst to be as smooth as possible.

If you’re new to HomeFirst and currently receiving treatment for a serious or ongoing health condition, you may be able to continue seeing your current provider for up to 60 days after joining the plan. Your provider must agree to work with HomeFirst, accept our payment terms, and share information needed to coordinate your care.

If you’re transferring from another MLTC plan, we’ll continue your existing services and care plan for up to 120 days, or until we work together to develop and implement a new care plan that meets your needs.

Things to Remember

  • Always carry your HomeFirst Member ID card. If you have Medicare, you should also carry your Medicare ID card.
  • Check that your provider is still in our network. Networks change. Look online or call us before your visit.
  • Get approval first. Except for emergencies, some services need approval before they start. Your care team usually handles this.
  • Tell us when things change. Need a new service? Different help at home? Just call.
