Leaving Elderplan for Medicaid Beneficiaries (HMO-POS D-SNP)

Being in this plan is your choice. You can leave whenever you want. This page explains when you can leave, how to do so, what we must continue to do for you until you go, and the few times we must end your membership.

Leaving a plan is sometimes called disenrolling. It means the same thing.

You can leave in any month of the year

You have both Medicare and Medicaid. Because of that, you do not have to wait for a certain time of year. You can leave in any month.

When you leave, you can choose:

  • Another Medicare health plan, with or without drug coverage
  • Original Medicare, plus a separate Medicare drug plan
  • Original Medicare, without a separate Medicare drug plan
  • If you qualify, another plan that puts your Medicare and most or all of your Medicaid benefits together

Your membership usually ends on the first day of the month after we get your request. Your new plan starts that same day.

There are also two set times each year when people with Medicare can change plans:

  • October 15 to December 7. Your new coverage starts January 1.
  • January 1 to March 31. You can make one change during this time.

Before you drop your drug coverage

If you drop Medicare drug coverage, stop getting Extra Help, and then go 63 days or more in a row without drug coverage that is as good as Medicare’s, you may have to pay extra later. Medicare calls this a late enrollment penalty. Call us first if you are not sure.

How to leave

To join another Medicare health plan: Sign up for the new plan. We will end your membership here automatically when the new plan starts.

To go to Original Medicare with a separate drug plan: Sign up for the new drug plan. We will end your membership here automatically when that drug plan starts.

To switch to Original Medicare with no drug plan: Send us a written request. Call Member Services at 1-800-353-3765 (TTY 711), and we will walk you through it. You can also call Medicare at 1-800-MEDICARE (1-800-633-4227), TTY 1-877-486-2048, and ask to be disenrolled.

What we must do until your last day

Until your membership ends, we still cover you. That is our responsibility, not a favor.

  • Keep using our network doctors and providers.
  • Keep using our network pharmacies or mail order.
  • If you are in the hospital on your last day with us, we cover that hospital stay until you are discharged — even if you leave the hospital after your new coverage starts.

You keep paying your share of costs until your membership ends.

When we must end your membership

  • You no longer have Medicare Part A and Part B.
  • You are no longer eligible for Medicaid.
    • If we learn you lost Medicaid, we will mail you a notice within 10 days.
    • You stay covered for 90 days from the date you lost Medicaid.
    • If you have not received your Medicaid back within those 90 days, we must end your membership.
  • You do not pay your medical spenddown if you have one.
  • You move out of our service area.
  • You are away from our service area for more than 6 months. Call Member Services at 1-800-353-3765 (TTY 711) before you move or take a long trip, and we will let you know whether that place is in our area.
  • You go to prison.
  • You are no longer a U.S. citizen or lawfully present in the United States.
  • You do not tell us the truth about other drug coverage you have, or you leave it out.
  • You gave us wrong information on purpose when you joined, and that information affected whether you could join. We need Medicare’s permission first.
  • You keep acting in a way that makes it hard for us to care for you and other members. We need Medicare’s permission first.
  • You let someone else use your member card to get care. We need Medicare’s permission first. Medicare may also ask the Inspector General to look into it.
  • Medicare charges some people with higher incomes an extra amount for Part D. If you are charged that amount and do not pay it, Medicare will end your membership in our plan.

Questions about any of this? Call Member Services at 1-800-353-3765 (TTY 711).

We can never end your membership because of your health

We are not allowed to ask you to leave for any health reason. If you think that is happening, call Medicare at 1-800-MEDICARE (1-800-633-4227), TTY 1-877-486-2048.

If we end your membership, you can complain

If we end your membership, we must provide you with a written explanation. We must also tell you how to file a grievance about our decision. You have the right to do that.
