Connect to Your Health Information

You can now manage your own health information using apps on your phone, tablet, or computer. This page explains what information you can access, how to get started, and how to keep your data safe.

What you can see

Elderplan for Medicaid Beneficiaries (HMO-POS D-SNP) covers your  medical, hospital, and prescription drug services. When you connect an app, you can see information we have about your care, including:.

Your claims and care records

• Claims for care you received, including hospital, outpatient, and pharmacy claims
• What we paid and what you owe, if anything
• Records of care from providers we pay in other ways, even when no bill was sent
• Dates of service and descriptions of the care you received

Your health information, if we have it

• Diagnoses and conditions (for example, diabetes or high blood pressure)
• Medications you are taking
• Allergies — medication, food, or environmental
• Lab results, such as blood tests
• Other clinical data we may have on file, such as vital signs, vaccinations, and procedures

Your coverage and drug information

• Drug coverage list, including preferred drugs and tier information
• Plan coverage details, including benefits linked to your prescriptions and services
• Provider and pharmacy directories

Prior authorization information

• Status of prior authorization requests — pending, approved, or denied
• Reason for any denial (available beginning 2027, except for drug authorizations)
