How Your Hearing Benefit Works
Know What’s Covered and How to Use Your Benefit
Original Medicare does not cover routine hearing exams or hearing aids. Your Elderplan plan adds hearing care as a supplemental benefit — including routine hearing exams, hearing aids of all types, and fitting and evaluation services — at no additional premium. Prior authorization is required before your hearing aid will be covered. If you receive a hearing aid without prior authorization, it may not be covered.
Routine Hearing Exam
Covered: One routine hearing exam every 3 years
Your cost: [$0] copayment
A routine hearing exam checks your hearing and helps determine whether hearing aids or other treatment may benefit you.
Hearing Aids & Fitting
Covered: Hearing aids, all types — up to [$1,300] for both ears combined every 3 years
Fitting and evaluation:[ $0] copayment, every 3 years
Your cost: [$0] copayment up to the [$1,300] limit
Prior authorization is required for hearing aids. Your physician or specialist must authorize the hearing aid before it is covered.
Your [$375] Flex Benefit can be used toward OTC hearing aides.

Important Reminders About Your Hearing Benefit
Things to keep in mind before your appointment:
| 3-year limit applies | Your routine hearing exam, hearing aids, fitting and evaluation are each covered once every 3 years. The limit resets on a rolling basis from your last date of service for each covered item. |
| [$1,300] combined limit for hearing aids | The [$1,300] maximum covers both ears combined. If your hearing aids cost more than [$1,300], you are responsible for the difference. Your Flex Benefit can help cover these costs. |
| Prior authorization is required for hearing aids | Always get prior authorization before ordering hearing aids. Authorization must come from a physician or specialist. Hearing aids dispensed without authorization may not be covered. |
| Always use your Elderplan Member ID card | Show your Elderplan Member ID card — not your red, white, and blue Medicare card — at all hearing appointments. |
| Hearing aid batteries are not covered by this plan | Hearing aid batteries are available through Medicaid using your Medicaid Benefit Card, not through this plan. Contact your local Medicaid office for more information. |
