Extra Support Beyond Your Benefits
Sometimes the help you need goes beyond doctor visits and prescriptions. As an Elderplan member, you have access to extra resources that can make everyday life easier — from finding stable housing and healthy food to getting to appointments or figuring out which programs you may qualify for.
Below are two ways we can help you connect. Choose the one that fits where you are today.
We work with our partner, Senior Planning Services (SPS), to connect you with community programs and resources that are not part of your plan benefits. SPS can help you with things like:
- Applying for assistance programs, you may qualify for
- Understanding the paperwork or letters you have received about public assistance or benefit programs
- Talking through your or public benefits and options when you’re not sure where to start
Available to: Members with Medicaid
If you have New York Medicaid, you may be eligible for support through the Social Care Network (SCN). This New York State program connects eligible individuals with community-based services and supports.
A Social Care Navigator will walk you through your options for:
- Housing and utility support
- Finding local services in your neighborhood for healthy food and nutrition services
- Transportation to career counseling or job interviews
- Care management to help coordinate your services
