Long-Term Care Services
Get the Care You Need, at Home
Most health plans cover doctor visits, hospital stays, and prescriptions, but they stop at the front door of your home. Elderplan Plus Long-Term Care (HMO-POS D-SNP) goes further. Your plan includes a full set of long-term services and supports (LTSS) — the day-to-day help that keeps you safe and independent where you want to be: in your own home and community.
Whether you need a little help with bathing and meals, a trusted caregiver by your side, a program to attend during the day, or skilled nursing care at home, your plan is designed to wrap around your needs — and adjust as those needs change.
What’s Covered at a Glance
Your Long-Term Care benefit includes the services below as Medicaid-covered benefits, plus the added benefit of coordination through your Elderplan Care Management Team. Most services require prior authorization — your care manager will handle that with you and your doctor.
