Your Elderplan Select (HMO-POS I-SNP/IE-SNP) plan includes Medical Nutrition Therapy (MNT). This preventive service provides personalized nutrition counseling to help manage certain health conditions, such as diabetes or kidney disease.

- Members with diabetes
- Members with renal (kidney) disease, not on dialysis

How to Get Started
- Ask your PCP or endocrinologist for a referral to a registered dietitian who participates with Elderplan Select.
