Therapeutic Leave
Therapeutic leave covers overnight stays at the skilled nursing facility (SNF) or assisted living facility (ALF) where you currently reside. Your Elderplan Select (HMO-POS I-SNP/IE-SNP) plan includes a Therapeutic Leave benefit so you can spend time with family, attend a special event, or simply have a change of scenery — without losing your bed or your benefits.

What Counts as Therapeutic Leave?
- Family events (weddings, holidays, birthdays)
- A short visit home or with a relative
- Medical or specialty appointments that involve an overnight stay
- Religious holidays or community events
How to Plan a Therapeutic Leave
- Talk to your facility’s care team as soon as you know the dates
- Call your Elderplan Care Team at 1-800-353-3765 (TTY: 711) to coordinate
- Confirm your medications — your pharmacy or facility will prepare what you need
- Plan transportation — your $1,000/quarter transportation benefit may help.
