How to Find the Plan That Fits You
Choosing a health plan can feel like a lot to handle. It does not have to be.
The right Elderplan plan mostly comes down to a few things about your life. This guide walks you through them, step by step. It takes just a few minutes.
Are you helping a parent or someone you love? You can use this guide for them, too. Just answer based on their life.
Three things that point to the right plan
You do not need to know plan names or health terms to get started. You just need to know a little about your day-to-day life.
Three things matter most:
- Where you live, do you live in your own home? In assisted living? In a nursing home?
- What coverage do you have? Do you have Medicare, Medicaid, or both? (Not sure? That is okay — we can help you check.)
- Whether you need daily help. Do you need help with things like bathing, dressing, cooking, or taking medicine?
Your answers point to the plan that best fits you.
Which one sounds most like you?
Read the list below. Pick the one that sounds closest to your life. It will point you to a plan to look at first.
- "I live in a nursing home." Look at Elderplan Advantage for Nursing Home Residents. It is made for people who live in a nursing home.
- "I live in assisted living, or I need that level of daily care." Look at Elderplan Select. It is made for people who need this kind of care.
- "I have both Medicare and Medicaid, and I live at home." Look at Elderplan for Medicaid Beneficiaries. It combines your Medicare and Medicaid into a single plan.
- "I have both Medicare and Medicaid, and I need daily help at home." Look at Elderplan Plus Long-Term Care. It brings both together and adds long-term care at home. HomeFirst may also be a good fit if you want to keep your Medicare separate.
- "I have Medicaid and I need long-term care at home." Look at HomeFirst. It covers long-term care at home through Medicaid.
Not sure which one sounds like you? That is very common. Skip ahead to "Still not sure?" below, and we will help.
Try our plan finder
Our plan finder asks a few simple questions and shows you the plans that may fit. You can use it on your own anytime. You do not need to sign in or share private details to look.
When you find a plan you like, we will help you take the next step. First, we make sure the plan is offered where you live. All we need is your ZIP code for that.
Will I qualify?
Some Elderplan plans have rules about who can join. For example, some are only for people who have both Medicare and Medicaid. Others are for people who need a certain level of care.
Here is the good news: you do not have to figure this out alone. When you find a plan that looks right, we will check with you to see if you qualify. There is no pressure, and you are free to ask.
Still not sure?
That is okay. Many people are not sure at first.
- Want to talk it through? Call us. A real person can help you find the right plan.
- Not sure what coverage you have? We can help you check your Medicare and Medicaid.
- Need to join a Medicaid long-term care plan? In New York, you usually start by calling New York Medicaid Choice. We can walk you through it.
Good to know
This guide helps you find a plan to look at first. It is a starting point, not the final word.
Before you join, we will confirm that a plan fits your needs and that you qualify. For full details on what each plan covers and costs, read that plan's Summary of Benefits and Evidence of Coverage. If anything here is different from those documents, those documents are correct.
