Medicare Advantage and Long-Term Care Plans
Elderplan Plus Long-Term Care (HMO-POS D-SNP)
A Medicaid Advantage Plus plan for New Yorkers 18 and older who have both Medicare and Medicaid and need long-term care support to keep living safely at home.
One plan, one card, and one care manager bring your doctors, hospital care, prescriptions, and home-based services together. Get extras like dental, a over-the-counter card for health items, a Flex Card, hearing aid coverage, and a no-cost fitness benefit.
Available in the Bronx, Brooklyn, Manhattan, Queens, and Staten Island, and in Nassau, Westchester, Dutchess, Orange, Putnam, and Rockland counties.
HomeFirst Managed Long-Term Care Plan (MLTCP)
A Medicaid Managed Long-Term Care plan for New Yorkers 18 and older who need long-term care services and support to continue living safely and independently at home.
HomeFirst works with you, your doctors, and caregivers to coordinate the long-term care services you need, including home care, personal care, adult day health care and other covered services.
Available in the Bronx, Brooklyn, Manhattan, Queens, and Staten Island, and in Nassau, Westchester, Dutchess, Orange, Putnam, Rockland, Sullivan, and Ulster counties.
Elderplan for Medicaid Beneficiaries (HMO-POS D-SNP)
A Medicare Advantage plan for New Yorkers who have Medicare Parts A and B and full Medicaid.
This plan covers your doctor visits, hospital care, and prescription drugs, all in one plan. You also get your own Care Manager for support.
Extra benefits include dental, vision, and hearing coverage. You also get a monthly allowance for over-the-counter health items, a Flex Card, transportation, and a gym membership through Silver&Fit..
You get comprehensive medical and prescription drug coverage, along with extra benefits that may include dental, vision, hearing, over-the-counter health items, transportation, and fitness benefits.
Available in the Bronx, Brooklyn, Manhattan, Queens, and Staten Island, and in Nassau, Westchester, and Putnam counties.
Elderplan Select (HMO-POS I-SNP/IE-SNP)
A Medicare Advantage plan designed for New Yorkers living in an assisted living community, nursing home, or other qualifying residential setting.
Elderplan Select provides coordinated care with support from a Nurse Practitioner or Physician Assistant and Registered Nurse who work with you, your doctors, and the staff where you live to help manage your health and care needs.
You get comprehensive medical and prescription drug coverage, along with extra benefits that may include dental, vision and over-the-counter health items and transportation.
Available to people living in a participating facility in the Bronx, Brooklyn, Manhattan, Queens, and Staten Island, and in Nassau, Westchester, Dutchess, Livingston, Monroe, Ontario, Orange, Orleans, Putnam, Rockland, Seneca, Suffolk, and Yates counties.
Elderplan Advantage for Nursing Home Residents (HMO-POS I-SNP)
A Medicare Advantage plan designed specifically for New Yorkers who live in a participating nursing home and may benefit from additional medical care and personal support right where they live.
Provides coordinated care with support from a Nurse Practitioner or Physician Assistant who works with you, your doctors, and nursing home staff to help manage your health and care needs.
You get comprehensive medical and prescription drug coverage, along with extra benefits that may include dental, vision, hearing, over-the-counter health items, transportation, and fitness benefits — with added support focused on your individual needs.
Available to people living in a participating facility in the Bronx, Brooklyn, Manhattan, Queens, and Staten Island, and in Nassau, Westchester, Dutchess, Livingston, Monroe, Ontario, Orange, Orleans, Putnam, Rockland, Seneca, Suffolk, and Yates counties.
