1. Members who are eligible to enroll in this plan must have full Medicaid and need community-based long-term care services for more than 120 days (e.g., in-home nursing, therapies, home aide, personal care, Consumer Directed Personal Assistance Services; private duty nursing; or Adult Day health care) but prefer and are able to live safely at home. If individuals are eligible for Medicaid only, they must also be assessed as nursing home eligible.
2. Members eligible to enroll in this plan must have Medicare and Medicaid, be 18 years of age or older, need community-based long-term care services for more than 120 days and be assessed as nursing home eligible.
3. Members in this plan receive a quarterly maximum amount of $950 to purchase eligible OTC items at authorized retail stores. The OTC card balance will be carried over to the next quarter but will expire at the end of the year.
4. Eligibility is determined by whether you have a chronic condition associated with SSBCI benefit (expanded OTC). Examples of SSBCI conditions include but are not limited to, Cardiovascular Disorders, Diabetes, Arthritis, Chronic Lung Disorders and Chronic Kidney Disease. There are other eligible conditions not listed. Standards may vary for this benefit.
5. Out-of-network/non-contracted providers are under no obligation to treat Plan members, except in emergency situations. Please call our Member Services number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.
6. No-cost gym membership is available at all fitness centers and select YMCAs in the Silver&Fit® network.
