How to Get Started
- Enroll: Enroll in one of our health plans (https://www.elderplan.org/enroll/) and get acquainted with eligible screenings in your plan.
- Schedule and Complete Your Screenings and Vaccinations: Make an appointment with your provider to complete your recommended screenings and vaccinations. Have your provider sign and stamp the form in the space provided.
- Submit your brochure/form:
- By mail using the enclosed pre-paid envelope
- By email at [email protected]
- By secure upload at elderplan.org/health-and-wellness/
- Earn Rewards: Receive a $25 reward for each eligible screening and flu shot completed by December 31, 2026.
