Elderplan Wellness Incentive Program

Elderplan is pleased to offer our members a $25 gift card for each completed screening, exam, vaccination or visit!

Your team at Elderplan knows how important preventive care is to your health and well-being. That’s why we are rewarding you for receiving the care you need with a $25 gift card for each eligible health-related activity you completed by December 31, 2023.

Eligible Screenings

Annual Primary Care Physician (PCP) visit.

  • PCP visits in-office, in-home, or telehealth count as annual visits.
Green stamp, Elderplan Wellness Incentive


The Wellness Incentive Program includes both the Flu shot and COVID-19 vaccination.

  • The COVID-19 vaccination incentive can only be earned once per lifetime for receiving the initial vaccine series.
  • The COVID-19 Booster and flu shot incentives can be earned once per calendar year.


If you have had a breast cancer screening between October 1, 2021 – December 31, 2023, it can count toward the 2023 incentive program.

Colorectal Cancer Screening

  • If you’ve had a Colorectal Cancer Screening within the last 9 years, it can count toward the incentive program.
  • The Fecal Occult Blood Test (FOBT) screening, which Elderplan offers at home, also counts toward the incentive program.

Retinal Eye Exam

Elderplan offers Retinal Eye screenings at a doctor’s office and at home. If you want a provider to visit your home, please call the Wellness Team at (718) 759-4413 to schedule an appointment.

How to Get Rewarded:

  1. Bring the Wellness Incentive Form to your next visit with your provider, or contact the wellness team to schedule your in-home screening (for colorectal cancer screenings and retinal eye exams only). The Wellness Team may also help locate a telehealth provider.
  2. Discuss and have the required PCP wellness visit, screenings, exams, or vaccination.
  3. Once completed, have your provider(s) sign and place their stamp on the Wellness Card.

Send your card as soon as it is completed, by December 31, 2023.
