At Elderplan, we understand the vital role preventive care plays in your health and wellbeing. That’s why we are pleased to reward you for receiving the care you need through our popular Wellness Incentive Program.
You can earn a $25 reward for each eligible screening, exam, vaccination or Doctor/Nurse Practitioner visit you complete within the specified timeframe.
Eligible Screenings for the Wellness Incentive Program
- New for 2026: Diabetes Screening
A hemoglobin A1c test to screen for diabetes and prediabetes. - New for 2026: Kidney Screening
Both an eGFR and uACR test to check kidney function. - Annual Primary Care Physician (PCP) Visit
In-office, in-home, or via telehealth - Flu Shot
A flu vaccination received in 2026. - Mammogram
A breast cancer screening completed between October 1, 2024 and December 31, 2026. - Colorectal Cancer Screening
A screening completed within the last 9 years. - Retinal Eye Exam
Retinal eye exam completed in 2026 - Hearing Exam
Hearing exam completed in 2026
Some screenings can be done at home.
Diabetes screening, kidney screening, colorectal cancer screening (via Fecal Occult Blood Test), and retinal eye exams may be completed in the comfort of your home. Call the Wellness Team at 718-759-4413 to learn more or schedule an in-home visit.
How to Get Rewarded:
- Bring the Wellness Incentive Form to your next provider visit
- Complete the required screenings and receive the vaccinations your provider recommends
- Have your provider sign and stamp the form in the space provided.
Send your card as soon as it is completed, by December 31, 2026.
