Get the Care you Need in the Comfort of Home

A Medicaid Advantage Plus plan for New Yorkers 18 and older who have both Medicare and Medicaid and need long-term care support to keep living safely at home. One plan. One member ID card. One dedicated care manager to help coordinate your care, prescriptions, and home-based services. Plus, you get extras like dental over the counter (OTC),  a Flex Card, hearing aid Coverage, and a no-cost fitness benefit.

Available in the Bronx, Brooklyn, Manhattan, Queens, Staten Island, Nassau, Westchester, Dutchess, Orange, Putnam and Rockland counties.
