At Elderplan | HomeFirst, we make it easy to get the support you need — even beyond the benefits provided by your health plan. You may be eligible for public programs that can help you access additional services and save money on health care, prescriptions, food, or household bills. We work with our partner, SPS Community Solutions, to guide you to the services you need.
What Is SPS Community Solutions?
SPS Community Solutions is our trusted partner offering free, one-on-one assistance to help you apply for public programs. The SPS Team supports you with applications, renewals, required paperwork, and follow-up — so you do not have to navigate these processes alone. SPS staff speak multiple languages and may offer in-home support, if needed.
SPS Community Solutions Can Help You:
- Check which programs you may qualify for
- Apply for public program benefits on your behalf
- Renew public program benefits on your behalf
- Help you gather required documents
- Track the status of your application(s)
Programs SPS May Assist With
If eligible and depending on your plan, SPS can assist with the following programs:
- Medicaid — Applying and/or renewing health care coverage for people with limited income and resources
- Medicare Savings Programs (MSP) — Assistance with paying Medicare Part B premiums and, in some cases, other Medicare costs
- Extra Help / Low-Income Subsidy (LIS) — Support to lower Medicare prescription drug costs
- Elderly Pharmaceutical Insurance Coverage (EPIC) — New York State program that helps older adults pay for prescription drugs
- Supplemental Nutrition Assistance Program (SNAP) — Helps cover the cost of food and groceries.
- Lifeline Program — A federal program that lowers the monthly cost of phone or internet service
- Low-Income Home Energy Assistance Program (LIHEAP) — Assistance with heating or cooling bills
- Pooled Income Trust — A pooled trust is a type of supplemental needs trust that allows New Yorkers to qualify for Medicaid without losing the benefit of income above the Medicaid limit. This helps you keep Medicaid and still use your money to pay bills.
