HomeFirst Provider Network

With HomeFirst, you continue to use your primary care physician and specialists.

We offer many home-, community-, and facility-based long-term care and health-related services.

This directory provides a list of HomeFirst’s network providers. To get detailed information about your health care coverage, please see your Member Handbook.

Introduction

We have network providers for all services covered under HomeFirst. You can still receive covered services from non-network providers for Medicare-covered services.

The “network providers” listed in this directory agree to provide you with long-term health care services. These services usually require prior authorization. To get detailed information about authorization requirements, please see your Member Handbook. If you have been going to one network provider, you are not required to continue going to that same provider. You must use plan providers except in emergency or urgent care situations. Please refer to your Member Handbook for guidelines.

If an out‐of‐network provider sends a bill directly to you, you should not pay it but submit it to the plan at the address provided below for processing and determination of enrollee liability, if any.

If you have a medical emergency

Get medical help as quickly as possible. Dial 911 for help or go directly to the nearest emergency room. In any emergency, you never need to contact any plan providers, even your own Primary Care Physician (PCP), for either permission (“authorization”) or referral. However, as soon as possible, you or someone else should contact your PCP or HomeFirst about your emergency. Your PCP or HomeFirst will need to be involved in following up on your emergency care. To contact HomeFirst, please call Member Services at 1‐877‐771‐1119, between 8:30 a.m. and 5:00 p.m., Monday – Friday. TTY/TDD users, please dial 711.

To request a hard copy of Homefirst’s provider directory, please call our Member Service Department at 1-877-771-1119 (TTY users should call 711) between 8:30 A.M. and 5:00 P.M., Monday through Friday. Homefirst will mail a hard copy of the provider directory within three (3) business days of your request.  Homefirst may ask whether your request for a hard copy is a one-time request or if you request to receive the provider directory in hard copy permanently.

If you request hard copies of the provider directory, your request will remain until you leave Homefirst or request that hard copies be discontinued.
