- Flex Card: $500/year for out-of-pocket expenses for dental, hearing, vision, and fitness

Elderplan Plus Long-Term Care (HMO-POS D-SNP)5
An excellent choice for Medicare beneficiaries who also qualify for Medicaid, require long-term care (LTC) services at home and prefer the ease and convenience of having all of their benefits under one plan – with no monthly premium. Elderplan Plus Long-Term Care provides a personal care worker to help with bathing, dressing, and meals.

- Community Rides3: Use your OTC allowance for rides with Uber or Lyft to visit friends, family, pharmacies, grocery stores, houses of worship, and more. You can also apply it toward gas, tolls, and parking.
