How do I join the Member-to-Member Community?
You can choose the option that works best for you:
- Online: Complete our secure and simple application. Start with your Elderplan or HomeFirst Member ID. This is the number next to Member ID on your Elderplan Benefits Card. Then we will ask questions about you and your interests.
- By Email: Download and fill out the application form and email it to [email protected].
- By Phone: Call our Member-to-Member hotline at 1-347-254-7700 (TTY: 711), Monday–Friday, 9 a.m. to 5 p.m.
- By Mail: Download and fill out the application form and send it to:
