Receive Enhanced Care in a Nursing Home

  • For Medicare or dual Medicare and Medicaid beneficiaries living in one of Elderplan’s contracted nursing homes.
  • Provides an additional level of care; a skilled Nurse Practitioner (NP) or Physician Assistant (PA) who will support and guide you by working with your physicians to create a customized care plan.
  • Available in the Bronx, Manhattan, Brooklyn, Queens, and Staten Island, plus Nassau, Suffolk, Dutchess, Monroe, Orange, Putnam, Rockland, Westchester, Ontario, Orleans, Seneca, Yates, and Livingston counties.

  • Bee iconnew fOr 2026! You can get up to 104 hours of friendly visits through The Helper Bees. A friendly companion can come for 4 hours at a time to read with you, play games, talk, or simply keep you company.

  2. Members of this plan receive a monthly maximum amount of $175 to purchase eligible OTC items at authorized retail stores. Your OTC card balance cannot be carried over to the next month. The OTC card is not a debit or credit card and cannot be converted to cash.[]
  3. Eligibility is determined by whether you have a chronic condition associated with SSBCI benefit (expanded OTC). Examples of SSBCI conditions include but are not limited to, Cardiovascular Disorders, Diabetes, Arthritis, Chronic Lung Disorders and Chronic Kidney Disease. There are other eligible conditions not listed. Standards may vary for this benefit.[]
