Whether you have Medicare and Medicaid or just Medicaid, Elderplan offers two Long-Term Care plan options that provide the services and support you want and need to remain safely in the comfort of your home.
HomeFirst Managed Long-Term Care Plan1
It is an ideal plan for eligible Medicaid beneficiaries who need assistance with activities of daily living but wish to keep their current prescription drug plan. This plan provides a personal care worker to help with bathing, dressing, and meals, plus attention from a dedicated care management team. Your dedicated care management team will contact your doctors to help develop a personalized plan of care and will also help arrange your medical visits.
Some additional benefits include:
- Home health care
- Personal Care Worker (PCW)
- Consumer-directed personal assistance services (CDPAS)
- Health Coaching
- Home-delivered meals
- Medical services such as dental, vision, hearing, and podiatry
This plan is available in the five boroughs of NYC, Dutchess, Nassau, Orange, Putnam, Rockland or Westchester counties
Elderplan Plus Long-Term Care (HMO-POS D-SNP)2
An excellent choice for Medicare beneficiaries who qualify for Medicaid, require long-term care (LTC) services at home and prefer the ease and convenience of having their medical, hospital, prescription drug and LTC coverage in one simple plan. This plan provides a personal care worker (PCW) to help you with bathing, dressing and meals, plus support, care coordination and attention from a Dedicated Care Management Team, all at no cost to you.
Some additional benefits include:
- INCREASED FOR 2026! $9503 per quarter for traditional OTC. Eligible members4 can also use their OTC allowance toward rent/mortgage, utilities, internet, groceries, home-delivered meals and community rides. Unused funds roll over to the next quarter.
- Community Rides. Eligible members4 can use their OTC allowance toward transportation, including Uber and Lyft, to visit friends and family, pharmacies, grocery stores, houses of worship, senior centers and more. It can also be applied to gas, tolls, and parking
- $500 Flex card per year for out-of-pocket dental, hearing, vision, and fitness expenses
- $2,500 toward hearing aids every year for both ears combined
- Freedom to choose any specialist in or out-of-network, giving you greater access to care5
- Vision: $600 toward eyewear

This plan is available in the Bronx, Manhattan, Queens, and Brooklyn, Dutchess, Richmond, Orange, Putnam, Rockland, Westchester and Nassau counties.
How to Get Started with One of Our Long-term Care Plans
Because the Department of Health (DOH) requires that you receive a health care assessment from one of our nurses to qualify for these plans, your first step is to establish eligibility by calling 1-866-384-3515 [TTY: 711], 8:30 a.m. to 5 p.m., Monday to Friday. An enrollment representative will confirm that you meet the eligibility requirements and help you determine if you require an assessment completed by the New York Independent Assessor (NYIA).
