  1. To be eligible for this plan, you must be 18 years or older, have Medicare and Medicaid, need community-based long-term care services for more than 120 days, and be assessed as nursing home eligible.[]
  2. Elderplan MAP is the only 5-star plan in New York State. This is the highest rating a plan can achieve. The New York State Department of Health rates and rewards MAP plans based on their performance on quality measures related to patient safety, preventive screenings, quality of life, satisfaction, and overall health status. Based on the most recent results from the DOH Consumer Guide published October 2023. []
  3. Members of this plan receive a monthly maximum amount (of $270 every month) to purchase eligible OTC items at authorized retail stores. Your OTC card balance cannot be carried over to the next month. The OTC card is not a debit or credit card and cannot be converted to cash.[]
  4. The benefits mentioned are a part of special supplemental program for the chronically ill. Not all members qualify.[]
  5. Out-of-network/non-contracted providers are under no obligation to treat Plan members, except in emergency situations. Please call our Member Services number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.[][]
  6. Hearing exams are covered once a year. Hearing aids and evaluation is covered once every three years.[]
  7. Please note, non-emergency transportation is now provided directly by Medicaid through Medical Answering Services (MAS). You will need to call MAS directly at 844-666-6270 if you reside in the 5 boroughs of NYC, Nassau, Westchester or Putnam or 866-932-7740 for Dutchess, Orange, Rockland, Sullivan and Ulster counties.[]
