2010 PLAN HIGHLIGHTS
The benefit information described here for each plan is a brief summary. For the full plan description, please refer to the Summary of Benefits located directly below each plan description.
Elderplan Classic I: Medicare Extra Needs (HMO) Brooklyn, Queens, Manhattan, SI: An Institutional Special Needs Plan for individuals who reside in Brooklyn, Queens, Manhattan or SI and need an institutional level of care, like a nursing home, on an ongoing basis, but who can be cared for in a community setting, like at home or an Assisted Living Facility.
- Still no plan premium (you must continue to pay Medicare Part B Premium)
- Unlimited $0 generic drugs, through the coverage gap, from our formulary1
- $25 preferred brand name drugs, but no gap coverage1
- $0 co-pay for in-network primary care physician
- You can choose from quality board-certified private practice network physicians
- Still not-for-profit health plan

Elderplan Classic I: Medicare Extra Needs (HMO) Bronx: An Institutional Special Needs Plan for individuals who reside in the Bronx and need an institutional level of care, like a nursing home, on an ongoing basis, but who can be cared for in a community setting, like at home or an Assisted Living Facility.

Elderplan Classic II: Medicare Zero Premium (HMO): Our most popular plan, it's for people who are eligible for a Medicare Advantage Prescription Drug Plan and don't want to pay a plan premium.

Elderplan Medicare Part B Premium Reduction (HMO)(with Part D): A plan for members who rarely visit the doctor or don't take many prescription medications.
- Reduces your Part B Premium by $40 each month (For enrollees who are responsible for paying their own Part B Premium payments.)2

Elderplan Medicare Part B Premium Reduction w/o Rx (HMO)(without Part D): A new plan for enrollees who want to keep their current Part D coverage.

Elderplan Medicare for Medicaid Beneficiaries (HMO): A plan designed for people with Medicare who receive state medical assistance as Qualified Medicaid Beneficiaries (QMB).
- You can choose quality board-certified private practice network physicians
- Low co-pay's for prescription drugs
- You will receive up to $480 a year ($120 per qtr.) of over the counter health related items3
- Between Elderplan, Medicare Advantage and State Medicaid, you do not pay for your medical services provided by in-network providers!4
- Still not-for-profit health plan

Elderplan Medicare Premium Benefit (HMOPOS): A new plan for individuals who want the freedom to receive Medicare covered benefits from outside of the Elderplan select provider network.
- Affordable co-pays for in-network services
- The ability to go out of-network for certain services (higher co-pay will apply)
- Maximum out of pocket cost is $1,000, with a $5,000 benefit Maximum for OON benefits
- You will receive up to $180 a year ($45 per qtr.) of over the counter health related items3
- Still not-for-profit health plan

Elderplan Medicare Extra Help (HMO): A new Medicare Advantage Prescription Drug Plan designed for beneficiaries who receive assistance with paying for their prescription coverage, referred to as Low Income Subsidy or LIS.
- You will receive up to $240 a year ($60 per qtr.) for over the counter health related items3
- Low cost sharing on prescription drugs (depending on LIS level)1
- $0 for your in-network PCP
- There are no referrals necessary for in-network specialists
- You can choose from quality board-certified in-network private practice physicians

Elderplan Medicare Plus Managed Long Term Care (HMO): A new plan for Medicare beneficiaries with full Medicaid who have chronic health care needs and are eligible to live in a nursing home, but would prefer and are able to live at home.

- Low cost sharing for Part D prescription drugs1

Elderplan Medicare for Nursing Home Residents (HMO): is available to anyone who requires nursing home level care and who resides in or agrees to reside in one of Elderplan's contracted Special Needs Plan network nursing facilities.
