Changes to Your Dental Benefit: What You Need to Know for 2026.

At Elderplan/HomeFirst, your health and well-being are always our top priority. We value your feedback and use it to guide us in improving the care we provide. That’s why we’re making improvements to help you get the dental care you need.

Starting January 1, 2026, we will be partnering with DentaQuest to help manage and provide your dental benefits. 

This means you’ll have access to a larger provider network—with more dentists to choose from, closer to home.


Browse the dental network for the following plans:

  • Elderplan Plus Long-Term Care (HMO-POS D-SNP)
  • Elderplan for Medicaid Beneficiaries (HMO-POS D-SNP)
  • Elderplan Flex (HMO-POS)
  • Elderplan Extra Help (HMO-POS)

DentaQuest has years of experience serving Medicare and Medicaid members in New York and offers a network more than twice the size of our previous one.

Good news: Your dental benefits will not change. You’ll continue to have the same coverage, now with even more options for care.

What You Need to Know

No Change in 2025

Continue using Healthplex providers through December 31, 2025.

You will receive a New Member ID Card

Between November and December 2025, you’ll receive a new card with DentaQuest’s contact information and a clear 2026 indicator.

Finding a Dentist

Use DentaQuest’s online provider directory to search for dentists or confirm if your current dentist is in-network for 2026. Because provider participation can change, always confirm with the provider before receiving services.

Ongoing or New Treatment

You may begin or continue care with a Healthplex provider until December 31, 2025. Starting January 1, 2026, all care will be managed by DentaQuest.

Why is Elderplan making this change?

We’re expanding access to quality dental care. DentaQuest’s larger network gives you more choices and convenience.

Do I need to take any action?

No. The transition happens automatically on January 1, 2026.

Will I be notified if my dentist is not in the new network?

Yes, we’ll notify you by mail between November and December 2025. However, we recommend checking DentaQuest’s provider directory or contacting Elderplan Member Services for additional support. You can also contact DentaQuest directly starting January 1, 2026. 

Will I get a new Member ID card?

Yes. All members will receive updated ID cards in late 2025 with DentaQuest’s contact information.

Will my benefits change?

No. Your dental benefits remain the same—just managed by DentaQuest.

What if I’m still in treatment after December 31, 2025?

You may qualify for continuity of care. Your dentist can contact DentaQuest for guidance starting January 1, 2026.
