Elderplan provides quality and affordable Medicare Advantage and long-term care plans for New Yorkers.

As a not-for-profit health plan founded right here in New York more than 35 years ago, our primary objective is ensuring that members of our community receive the care and support they deserve.

Elderplan offers a variety of Medicare Advantage plans tailored to fit the changing needs of Medicare and dual Medicare and Medicaid beneficiaries at every level of health. We are proud to continue to have a 5-star rating and be the only 5-star Medicaid Advantage Plus (MAP) plan in New York State*.

This plan covers Medicare, Medicaid, long-term care, and prescription drugs. These ratings demonstrate that our members appreciate the best-in-class care they receive each and every day, and that differentiates Elderplan. We are dedicated to ensuring that we offer high-quality plans at every level of care.

We offer plans with no monthly premiums, $0 co-pays for in-person or telehealth primary care and specialist doctor visits, comprehensive dental, an OTC benefit for health-related items and groceries (which includes home-delivered meals), $0 for transportation to medical appointments, free gym membership, and so much more! Plus the added benefit of being able to join the Member-to-Member Community.

The Value of Being Part of an Integrated Health System

When it comes to your health, things can change day-to-day. That’s why another great advantage of being an Elderplan/HomeFirst member is that we are part of MJHS, a not-for-profit health system founded in 1907, that includes: MJHS Home Care, MJHS Hospice and Palliative Care, as well as Isabella and Menorah Centers for Rehabilitation and Nursing Care.

So should you require access to additional support over time, and choose to receive services from MJHS, the Elderplan team can work together with their colleagues from across the system to better coordinate your care and make it easier for you.
